For proper treatment, it is necessary for the therapist to create a file. This contains your personal data, notes about your health and data about the examinations and treatments performed. The file may also include data that is necessary for your treatment and that De Dorsslede, after your explicit permission, has requested from another healthcare provider such as the general practitioner.
De Dorsslede handles this data with care and considers your privacy to be of great importance. We ensure that no unauthorized persons have access to your data. Only your treating therapist has access to the data in your file. In doing so, we comply with the rules as laid down in, among others, the GDPR (General Data Protection Regulation), the WGBO (Medical Treatment Agreement Act) and the professional code of the professional associations. In this privacy statement, we explain what we do with your data.
The obligations of De Dorsslede
De Dorsslede is responsible for the careful handling of your personal data. This means that:
We only collect your data for specific purposes:
· Care;
· Effective management and policy, such as financial administration;
· To comply with a legal obligation
Your personal data is carefully and securely stored electronically and on paper.
De Dorsslede has professional secrecy to handle your personal data confidentially.
Your personal data is not stored longer than necessary for good healthcare. For medical personal data this is 20 years.
Your rights as a client
You have the following rights:
· The right to know which personal data of yours is processed and the right to inspect and copy that data.
· The right to correct, supplement or delete your personal data.
· The right to limit the processing of your personal data, for example if the data may be incorrect.
· The right to object to the processing of your data.
You can inform De Dorsslede in writing if you wish to exercise these rights. People aged 16 and over who wish to exercise their rights must submit the application themselves.
Sharing your data
The data from your file is used by De Dorsslede for the treatment, but can also be used:
· To inform other healthcare providers or teachers. This is only done with your explicit permission.
· By another care provider/therapist in the absence of your own therapist. This is only done with your explicit permission.
· For anonymised use during peer review, such as supervision or intervision.
· For the financial administration and client registration. In order to guarantee your privacy in this as well, De Dorsslede has concluded processing agreements with the parties that do this.
Exchange of data
After you have given your explicit permission, De Dorsslede will exchange relevant personal data via secure email/App, by post or telephone. If you share confidential personal data with De Dorsslede via the App and/or email, this is your own responsibility.
Changes to the privacy statement
De Dorsslede can always change this privacy statement. A current version of the privacy statement is published on the De Dorsslede website. It is advisable to consult the privacy statement regularly so that you are aware of any changes.
Question or complaint
Do you have a question or complaint? De Dorsslede would be happy to discuss this with you. De Dorsslede would like to resolve a complaint to your satisfaction. If this does not work, you can also file a complaint with the Dutch Data Protection Authority or the complaints officer of the WKKGZ (Healthcare Quality, Complaints and Disputes Act).
